I hereby verify that I am working as an independent contractor responsible for my own insurance and any taxes. I hereby verify that all of the above information is complete and accurate to the best of my ability. I understand that the submission of this application does not guarantee that I will receive assignments from A City Suburban Service, Inc., but that I will be contacted by a representative of A City Suburban Service, Inc., if they choose to utilize me as an independent vendor. If accepted as A City Suburban Service, Inc., independent vendor, I understand that the following documents will be required: 1) Executed A City Suburban Service, Inc. Contract with supplements; 2) Executed Confidentiality Agreement; 3) Certificate of Insurance (evidencing required coverage to include commercial general liability coverage, errors & omissions coverage, and personal property coverage); and 4) Completed and signed Form W-9.
